Applying for admission to Saint Mary’s School is the first step in what we hope will become a lifelong partnership with your family. Over the last seven decades, thousands of satisfied families have made the same exciting decision!
Below you will find our application process outlined. The admissions committee will review your completed application on a rolling basis as space is available. We will notify you promptly once an admission decision has been made.
Each form may be sent in individually as it is completed. Forms can be submitted via mail, email, fax, or delivered in person to the school office.

1. Fill out Application
Complete the application form and return it to Saint Mary’s with the application fee of $25.
2. Complete Records Request
Complete the top two lines of the Records Release Request, and sign and date it at the bottom. Return the form to Saint Mary’s, and we will request your student’s records from his or her former school.

3. Additional Required Admissions Forms
Student Evaluation Form: Print the appropriate form for your child’s current grade level. Fill out the top portion and give it to your child’s current teacher to complete and return directly to Saint Mary’s.
Physical Form: Print the Catholic School Health Report. Complete and return the first page. Give the second page to the child’s pediatrician to complete. The physician or the parent may return the form to Saint Mary’s.
Eye, Ear Dental Form: Georgia state law requires that all children age four and older must have this screening. The pediatrician will complete this form. The form may be returned to Saint Mary’s by the physician or by the parent.
Medication Permit Form: This form is required in order for Saint Mary’s School to administer any medication to a the child. Even if your child does not currently take medication, it is a good idea to have this form on file for your convenience, just in case there becomes a need for a child to have Tylenol, cough drops, etc.
Parish Verification Form: This form is required for all Catholic families and must be completed by the parent. If you are a parishioner at Saint Mary’s Catholic Church, retuen this form to the school. IF you are a member of a different parish, please take the completed form to your church office to sign and send to the school.
Birth Certificate: A copy of the child’s birth certificate is required as part of his or her admissions folder.
Immunization Records: The child’s immunization records are required for admission to Saint Mary’s. You may request a copy from the child’s pediatrician.
Baptismal Certificate: A copy of the child’s baptismal certificate must be provided by Catholic families only.
4. Entrance Testing
Saint Mary’s School requires entrance testing at certain grade levels. Please contact the Admissions Office to determine if a testing date will need to be scheduled for your student.
5. Visit Saint Mary’s
We can’t wait to meet your family! At any point in the Admissions process, we would love to set up a visit with you and your child. You are more than welcome to visit before all your admissions forms are completed.
